Impact

For more than 30 years, the Center for Community Health has partnered with communities to advance health equity through community-led solutions. The 2025 Impact Report highlights the collective impact of these partnerships and the progress made across our programs, initiatives, and collaborations through the end of 2025. None of this work would be possible without the dedication of our community partners, funders, collaborators, and staff.
